  • Multiple Languages for Customer Service and Support

Range of Dental Plans Offered

Liberty Dental Plan offers Medicaid dental plans in Florida and Nevada. It also offers MediCal dental in California. Its MediCal dental plans include free transportation for its PHP members.

Health insurance Exchange dental plans are available from Liberty Dental Plan in California, Florida, Missouri, and Nevada. These plans are great for people looking for affordable, comprehensive coverage. They are part of the health insurance marketplace set up by the Affordable Care Act.

In California, a Covered California Small Business Plan is also offered to small business owners on the health insurance marketplace. If you’re in Florida or California, you can also buy private dental plans through Liberty Dental Plan.

Liberty Dental Plan’s plans are typically Dental Health Maintenance Organizations (DHMOs) or Preferred Provider Organizations (PPOs). These plan types indicate the kind of network coverage offered. DHMO plans only offer coverage for in-network dental care, while PPO plans offer some coverage for out-of-network dental care. Keep in mind that even with insurance coverage, out-of-network care tends to have higher out-of-pocket expenses for you.

Multiple Languages for Customer Service and Support

Liberty Dental Plan provides information and support in many languages, including Spanish, Chinese, and Vietnamese. Members can also receive help and forms in other languages as needed.

The Bad

Limited Availability

Liberty Dental Plan only offers programs in California, Florida, Hawaii, Illinois, Missouri, Nevada, Jew Jersey, New York, and Texas. If you’re not in one of those states, you’ll need to look for another dental plan provider.

The Bottom Line

If you’re looking for Medicaid dental coverage, a dental plan from the health insurance marketplace, or a private dental plan, Liberty Dental Plan may have an option for you depending on where you live.

The other perk of working with Liberty Dental Plan is that you can receive documents and support in your preferred language. The wide range of language offerings makes Liberty Dental Plan accessible by many in the United States.

If Liberty Dental Plan isn’t available in your area or doesn’t offer a plan you need in your area, you’ll need to consider alternatives.

Horrible. Customer service is a joke. They can't help you . No root canal specialist. The list they gave no one takes the plan. Two dentists offices stated to me liberty is a "NIGHTMARE". Grievance done still no results. I was told someone would contact me over two wks ago. They never did. I've called numerous times to be told again and again someone would call me. I'm reporting them to BBB and the dental association.

Denied WellCare plan coverage as primary provider because we have a separate plan with Cigna we pay for separately. Dentist insurance agent said first time seen in her 20 years at the office., she has never seen primary coverage denied, even though it is your Medicare advantage provider. Websites, accessing claims, non existent. Avoid Liberty Dental or at least don’t rely on timely; if any payment. Just like the judicial system, kick the can down the road, and hope everyone forgets.

This co is the worst. Most unprofessional customer service reps. Walked around wit broken tooth for three weeks they denied claim didn’t like it was sent in two pages needed to be in one. Then 6 months later they pulled from dental clinic and will not approve second cleaning cause X-rays will need to be done elsewhere. This con is a joke and I’m about to leave Aetna’ for this and go to where they have reputable drs and clinics

Liberty Dental Plan zero ratings to equal there payment plan all claims denied but don't be late with your payments to them they don't like to pay i needed complete extractions ,immediate denture ,full dentures in 2017 claim was denied file internal appeal to the same dictator that took 1 year denied file #2 appeal also another year ,granted for fare hearing that i won in 2019 by a judge order now is 2024 still no dental work 3 hr on phone with anthem bc & liberty rep many time in the week day after day as they are all working remote & all are first name of people you will never talk to again ,calls get disconnected try again all new agents & robots sending you in circles as they wear you down to exausion . after all said yes don't worry you will get all we agreed to in judges order so just when you think all confusion & drama was settled that day . liberty company clerk sussy storms calls Friday night after 6 pm say denied forget about judges order or anything everybody told you in the past or today . i am the boss & your claim is denied go file appeals again final answer . i said send me in writing letter & don't call me on burner phone being a rude dictator barking orders & denials @ me & you are not a claims adjuster or a judge & cant overrule what was already ordered in judgement dated 2019 . she Hung-up on me & cant callback to burner-phone. there provider list is outdated & 99% of listed vendors are no-longer even when i get letters in mail from liberty this is your new dentist unless you change in 30 days . so ever 30 days i get another letter & when i call that vendor they say we don't deal with them they don't like to pay .now in 2023 i get letter sending me back to my first dentist from 2017 metropolitan dental associates where the problems began as the dentist bully's customers to get copay for what you need in full diagnose but they know what the ins company wont pay so they wanted me to pay $2500 EXTRA when my plan is a no copay disability plan paid by NYS & NYS will never deny me any medical treatment i need so i told dentist to submit the full diagnoses & plan of care send full diagnoses to liberty dental ins. & give me an estimated bill especially if you want $2500 copay from my pockets . my last visit to metropolitan dental associates in 2022 asking for re evaluation to get update approval of 2019 i was told liberty don't want us to treat you any more so here is a referral to nyc dental school don't come back here ever again & i said i got you $2500 more money that took me 3 years fight for & you are a for-profit business that just got all the money you wanted & now you turn me away to a dental school . so i say these 2 businesses are scammers in violation of the Hippocratic oath first do no harm as run out the clock on your policy's & approvals to where premiums are paid but claims are denied by clerks instead of licences claims adjusters & then your told your not covered we don't have to honer any past claims pending with us . even-tho my plan is automatically renewed every year like clock work so now is 2024 i still need the same dental treatment as i did in 2017 so in short word i would call that dictatorship orthodontia consumer & taxpayer FRAUD

Like many other reviewers I have had bad experiences with Liberty Dental here in South Florida. It is hard to find a general dentist and very difficult to find specialists oral surgeon and endodontists. I’m on a Medicare Supplement HMO plan. The coverage they sell is great but the dentists all say they are losing money on Liberty patients! Pre authorization is required for everything, and it is difficult to submit pre authorization to Liberty and get them to approve. I lived with dental pain for 9 months in 2023 before I got them to approve what started out as a crown and developed into a root canal because of their repeated denial of treatment.

I got this dental insurance through health care advantage Devoted. They give me $3000.00 for comprehensive dental care but when my dentist submit claim less than $3000.00, they denied the claim. I end up with massive dental bill. Do not take any health care advantage offers dental insurance through Liberty Dental.

The absolute worst! Like other posters I wish I could leave zero Stars. They have providers listed on their website that are no longer at the practice and are dropped without notifying the dental office , as in my case as I'm trying to get a root canal. I went to one of the listed practices that was given to me by customer service ( I double checked on their website)but they were dropped before my referral went through, (unbeknownst to the practice) sìnce it's be done by snail mail. In addition to providing names of Specialists that are no longer at the locations, they provide names of dental practices that no longer take the insurance. Calling customer service is useless because they give you the same incorrect information that is on the website , it is like speaking to a parrot. Now I have been suffering due to being unable to get a root canal even though I went to one of their listed providers that they conveniently took off their website before I got an approval for the procedure and as I stated when I called customer service I was given four names of endodontists that were no longer at that practice. Now I have to start all over again to try to find an endodontist that will accept this insurance as well as wait weeks for the approval. In the meantime I'm in excruciating pain as an infection rages through my body

The only reason there's so called customer retention is because people aren't given a choice. Liberty Dental is incompetent and should be shut down. Liberty Dental sent me all over the state of Florida chasing after an oral surgeon for which I have a referral. Liberty Dental sent me to an office with no electricity and a leaking roof, where homeless people were finding shelter (bless them) and to offices who weren't oral surgeons at all. It's really astonishing. This is insurance in name only. Unfortunately, Liberty Dental prevents people from accessing very limited opportunities for free and sliding fee dental care, because they are "insured".

If I could give 0 stars I would! This dental insurance is a joke! They seem to have great coverage and quite a few providers in network but that all ends on their website! When dental providers quit accepting them, they don’t take them off their site so you think you have a number of choices! Wrong! Then they require the dental practices to get pre-authorization for almost everything. It takes about 2 weeks for review and then they (according to a number of dental practices I’ve contacted), don’t pay their portion! There are 0 specialists in network currently yet their site lists 33 near me! You ask them to find you a provider and they say they’ll call you back with results but you never hear back from them! They are a rip-off! Stay clear of Liberty Dental!

I am compelled to share my infuriating experience with Liberty. As a concerned parent, I believed that investing in dental insurance would provide a safety net for my family's dental needs, especially emergencies. However, when my two-year-old child required urgent dental treatment, this company revealed its true colors by callously denying coverage. The incident unfolded when my little one encountered a painful dental emergency. Panicked, I rushed to find immediate dental assistance, only to be greeted with an icy reception from our trusted dental professional. They informed me that our dental insurance company refused to cover the required treatment for my toddler, citing a ridiculous excuse: they have doctors that can only do another treatment that my child doesn’t need, therefore ineligible for emergency coverage. My child needs a specific treatment that there doctors aren’t able to do. I was dumbfounded by the sheer lack of empathy and concern exhibited by Liberty. Are dental emergencies somehow less important for children under two? It is unconscionable to subject a young child to such pain and discomfort, all because of a preposterous policy that disregards the urgency of dental emergencies. When I contacted their customer service, I was met with indifference and a disheartening lack of understanding. Their representatives offered no viable solutions or alternatives, leaving me to bear the brunt of the financial burden alone. It is utterly deplorable that a company supposedly dedicated to dental health would deny coverage for essential and immediate treatments for vulnerable children. Additionally, throughout my unfortunate interaction with Liberty, their communication was frustratingly opaque and non-transparent. They failed to clearly outline the limitations and restrictions of their coverage for young children, leading to misleading expectations and wasted time. As a result of this experience, I strongly advise anyone considering dental insurance to steer clear of Liberty . They may advertise comprehensive coverage, but in reality, they are nothing more than heartless profiteers, who prioritize their own bottom line over the well-being of their policyholders, especially innocent children. I am now left with a bitter taste in my mouth, not only due to the financial burden I had to shoulder but also because of the emotional distress caused by this heartless insurance company. Spare yourself the anguish and find a dental insurance provider that genuinely cares for its policyholders and upholds their promises. Unfortunately, Liberty falls drastically short in that regard. Please, do yourself and your loved ones a favor by avoiding Liberty. Your peace of mind and the well-being of your family's dental health are too valuable to be entrusted to a company that fails to prioritize its policyholders, especially in times of emergency.

It should be rather obvious from all of the reviews that Liberty is HORRIBLE. I will add that if you have any choice, stay away from Liberty. DentaQuest seems to be the product of choice for Medicaid, though it's not so great either. My experience with Liberty is similar to what many others have said: Dentists and providers dropping Liberty due to problems with getting claims filed and settled, constantly changing providers, claims not being processed properly, having to pay out of pocket far more than I should have to, and a lack of communication. In particular, I had to try for 6 months to get a proper referral from the primary dentist to get oral surgery. The dentist had to submit the referral 4 times to get Liberty to accept it and send it to us. When we finally got that, scheduling with the nearest oral surgeon (which was over an hour of driving distance away!) was a 2 month wait just for the initial consult, followed by another 2 months for the surgery. And then just before the surgery the oral surgeon stated that they dropped Liberty and wouldn't be filing our claim for us. So we had to pay out of pocket and file the claim ourselves, only for the claim to be denied because the surgery required a preauthorization that was never requested by the surgeon. We are still fighting about this and find it completely unacceptable.

Liberty Dental is part of my Florida Oscar Medicare Advantage benefit services which I opted for in 2022. There were a lot of network dentist within 5 miles of my home when plan started but as I checked further determined most associated with large chain dental offices. It was difficult to get anyone to actually answer the phone to make appointments. I finally found one good and small dental office in Fort Lauderdale, FL near my home and got 1st appointment in April 2022. They were all very nice staff and did x-rays and check- up and made some recommendations for dental work but cautioned that they needed to get pre-authorization from Liberty before would do any work since they had so many problems getting paid in past. Liberty did make the approval in writing to me and dentist but cautioned it did not guarantee approval once work done. Within a week of my dental visit I got another written notice from Liberty that the dentist I went to is no longer in the plan and the work authorized was for that dentist only and I would need to select another dentist in the Liberty Plan and start process over. I checked the list of original dentists on plan in January and found there were very few left. Either dentist dropped out of plan or stopped taking new patients. Originally there were at least 50 dentists within 5 miles of m home and now there were 3 left. That should tell me enough about why these dentists stopped taking any more patients. I will not use Liberty again even though my Oscar Medicare Advantage plan still offers it with a $2000 annual limit. If you can't get dental work done, then Liberty keeps the profits from what Oscar provides to them in the contract to provide dental service to their members. I returned to my private dentist to get my dental work done and had to pay out of pocket. I let Oscar know of all the problems I had with Liberty Dental and not to continue to contract with them since it is a waste of money and no benefit to the Oscar member as well as hurting Oscar's reputation for offering such a poor dental plan. I only used $64.00 of the $2000 limit in 2022 due to Liberty Dental problems.

They are the worst---oh yes, my dental company takes Liberty Dental, BUT UNLESS the dentist is contracted with them--DENIAL-- Makes me so upset as I do NOT want to find a dentist SOMEWHERE that is contracted with them, it's NOT FAIR. Yes our dental company IS on their plan, BUT even though my dentist works for my dental company, I was denied as the dentist who did my work was NOT on "their plan" I don't want to have to "TRY" a different dentist as who wants to start all over with going over ALL my history and having to have a stranger work on my mouth when I have been with the same place for about 20 years. I understand if one retires and I HAVE to switch to another dentist in the same office, but at least they know me and can consult with the previous dentist until he leaves. My dental office is WONDERFUL and is so good about working with me on payments and they treat me like a PERSON, not just another CUSTOMER. Liberty Dental is just another insurance co. that is only concerned about them making money, not us as consumers. IF Blue Cross Advantage doesn't find a better Dental Company, I will have to switch to one that does.

My insurance agent recommended I replace my Medicare with a BCBS replacement policy. I'm not a big consumer of health care and the dental coverage that is subcontracted with Liberty Dental was the one thing I needed. But after months of effort, many, many hours of effort and massive frustration, I have not been able to get a claim submitted into their system. Both BCBSNC and Liberty Dental are dismissive, avoid answering the phone, and refuse to call back from phone messages. I'm planning on taking them to small claims court, filing a complaint wit the state insurance commission, and -of course- changing insurance companies next year.

Trash dental insurance that doesn't actually cover anything. I was told we had a zero deductible and they would cover 80% of my daughter's dental work. She needed some fillings well guess what. They will after we have paid $17000 out of pocket yes 17k! I said I thought our deductible was zero well it is! BUT This is called and out of pocket maximum which they require before they will cover anything. What a total rip-off. Had I known this I would have added her to my own dental plan they made this sound like a good deal now if I change plans she would have to wait 6 months before they would cover the fillings. This company is a joke and they should be shut down for ripping people off with out right lies.

Absolutely disgusted with this plan and their services! My next step is contacting better business bureau! My 15 year old needs braces. Yes our plan is through medicaid so obviously I can not afford such high cost payments in dental care for his braces. He RECIEVED a referral for over crowding. I called liberty to see who he can go through. I was told braces are covered and that the closest Ortho was over an hour away. I take my son this past Monday to be seen. The Ortho called me today to tell me that Liberty had denied his claim. I call back the insurance company to see what the reason was for. I was told that liberties personal dentist reviewed the claim and because the problems with his over crowding do not keep him from being able to chew is why he was denied?!? You have got to be kidding me!!! So now my child will have crooked teeth because he is able to chew? This insurance company is absolute TRASH!!! Go else where! They will tell you they cover something just to turn around and deny a claim for a child!!!

Worst ever to deal with nothing made clear ! 3 months trying to get a claim filled they don’t tell you they chose what they will pay what they choose out of your dental care , so you won’t get your 1000 dollar coverage they get to keep the rest of what they won’t use for you ! No call backs wants your past records if you are new to them ! But doesn’t tell you that not even the dential office manage who called them on my behalf to summit the claim AGAIN trying to get reimbursed! I will pray for those who work for them for what they do to people 🙏

The staff that they employ at Liberty Dental in customer service are not even qualified to manage their own bodily functions. They no less then I do about the plan. They are inept, incompetent, and ignorant. Even their hard copy reply are not accurate. I have Liberty through Aetna. Liberty makes Aetna look very bad. Aetna needs to either take control and answer customer inquires or stop offering dental. I now have a contact at Liberty who is competent. The people in customer service at Liberty should have to pay Liberty for letting them use their space.

The skill in this company is limited to jerking aroud the people they insure. After 6 months and a 100 phone calls of them trying to find a provider I am still awaiting services.. They have played every game in the book. Referring me to dentists who do not take the plan. Referring me to dentists who do not perform the procedures I require. Referring me to dentists more than 100 miles away. Scam isn't too strong a term.

My daughte has an impacted #11 canine angled at 45 degrees. Due to the eruption path of #11 primary tooth "H" will likely be retained and there is a possibility of root resorption of #9 and #10. The dentist said it is in her best interest to have orthodontic treatment in conjunction with oral surgery to expose and bond #11. BUT THEY DENIED THE CLAIM AND THE APPEAL. This dental insurance is a joke. I've put in a complaint with Medicaid and BBB.

They wait until days before procedures to let you know theyve denied it. They think they know your health better than you do and want to decide what's right for you even tho YOU AND YOUR DOCTOR know your plan better than some weasle at a desk pushing no on every claim. Absolutely worst dental insurance I've ever experienced. Total trash. Guess you cant expect much from something that's government funded. TRASH

I wish I could give ZERO stars. This insurance is the worse. They accept very few dental providers and will not approve any out of network for 75 miles!!! Who can drive 1 hour for an appointment! This is absurd. I have multiple children, and have a job. There is no way possible they can expect people to drive an hour for an appointment, an hour + for the actual procedure and then an hour home In pain after a procedure. Absolutely ridiculous. They will not work with you. I am highly upset!

Even did a 3way chat with the dentist and have been denied at every step. Denials such as u can’t get a partial but a removable denture. Which is a partial. Just the worst dental ever. No one seems to accept the insurance and the places that do will not answer the phone or are closed. It has been nine months now and I still haven’t been able to make an appointment. I know COVID plays a huge factor in why but it’s an emergency. There had to be someone out there that can help me. Out of 30 Dental offices liberty provided to contact only two have answered my calls and they weren’t accepting new patients 😒 That’s INSANE!!!

This is truly the worst company I've ever had to deal with. Their provider was over 20 miles from me. This is NOT acceptable.

Crappy coverage. Was just told they’ll pay for a filling, but not the lidocaine to numb it before drilling!? WTF? Previously, they grudgingly agreed to pay $34 of my total $2000 root canal bill. It wasn’t worth the trouble of filling the paperwork, so the dentist just waived the $34.

I am manic depressive bipolar. I had a front tooth removed. I tried getting a replacement. I was denied. I did an appeal stating I had a previous suicide attempt but that didn't matter. I even provided dental evidence about the harm of not replacing a tooth. I was still denied. It's part of my Medicare Advantage Plan and I'm on a fixed income.

Liberty spends more time trying to figure out how they can deny a claim then pay one. This is one of the worst dental plans I have ever seen. I would absolutely NOT recommend them to anyone. Horrible, horrible, horrible!!!

When I search for a dentist, it turns out they only take Liberty for children. What good is that for someone with a Devted HMO? How about a separate listing for adults? I'm sure it can be done ,.

Want to find a dentist?? GOOD LUCK!! They tell you straight up they can't find anything...and they won't help. Their insurance sucks and you can't use it...hope you don't need a dentist...

It's a Medicaid dental insurance and it is not very good. Not many providers to choose from and they don't cover really anything.

I am contacting the news! What total fraud! No periodontist within 150 miles! Yet they cover this? How is this company getting away with this?

For two years I've been trying to get my teeth pulled. Still trying today. Liberty Dental is so limited it's don't work unless you have good teeth.
